I created it using wordle.net.

In order to get the various font sizes, I used 6 generations of surnames, then weighted each surname by how many ancestors with the name were in the 6 generations. For example, my surname is Seaver, so I put six Seaver names in the list of names for the Wordle. My mother's maiden name was Carringer so I used 5 Carringers in the list of names. All the way down to the 6th generation where each new surname was listed only once.
