My friend, John Finch, gave a presentation on World War I today and showed this database and the form with questions. I found this page for my great-granduncle, John Henry Richmond (1865-1947) in the database.
If you have a male residing in Connecticut in 1917 and born in the 1850 to 1900 range, there may be a form for him.
