This record was copied by the Chester, New Hampshire town clerk in 1905 to a vital records card that was submitted to the New Hampshire vital statistics office in Concord.  The original record is in Volume 1, page 115 of the Chester town record book, which is not available on FamilySearch or Ancestry.com.  It is available on FHL microfilm 1,001,049.

John Underhill is my 5th great-grandfather who married Hannah Colby (1745-????) in about 1767, and they had six children between 1770 and 1779, including my 4th great-grandfather, Amos Underhill (1772-1865).
